  • Health Insurance

    PHS offers health insurance coverage to eligible team members and their eligible family members using pre-tax dollars. You and PHS share in the cost of the coverage.

    Vision Insurance is included in select Health Insurance plans.

  • Dental Insurance

    Optional dental coverage is available for eligible team members and their eligible family members using pre-tax dollars. Choose from three different dental plan options.

  • Life Insurance

    Full time employees up to age 69 are provided with life insurance in the amount of $25,000. Employees age 70 and up receive half this coverage amount. employees who participate in the company’s health insurance plan are eligible for additional life insurance in the amount of $10,000.
